Dr Toscova is not accepting new patients at this time. 

APPOINTMENTS

 

If you are unable to keep your appointment, please call to cancel as soon as you know at least 24 hours prior to your appointment. Insurance and cancellation fee will be billed for missed appointments which were cancelled with less than 24 hours notification. 

CONFIDENTIALITY

Due to HIPPA confidentiality guidelines, I am unable to disclose anything pertaining to your treatment without you signing the Release of Information Form (ROI), available by request, per person. 

Please consult your insurance to see if you need to obtain prior authorization before your first visit.

I am a provider for the following insurance companies:

Lovelace, Presbyterian, United, Aetna, Tricare / Triwest, ConnectCare, Molina

Blue Cross Blue Shield (There are several different groups, so you need to check that I am a provider for your particular BCBS)

Medicaid / Medicare

Prior to your first session, forms along with a digital/printed copy of your insurance card is required.

120.00 /hour per 45-55 minute session 

 

Copays are due at each therapy session unless arrangements are made. I file and bill your insurance but do not bill/make statements for copays. If you do not have insurance or do not plan on using it , you may make a payment each time you come in or you may choose to make monthly payments.

 

Cancelation for subsequent therapy sessions require 24 hours notice or you may be subject to a $60.00 cancelation fee.
